What are the major strategies to reduce the risks of cannabis smoking?


If you smoke tobacco then you are not going to damage your health further by smoking cannabis. However if you wish to reduce your smoking activities then smoking cannabis with a high THC content is sensible as you will smoke less.

Mostly cannabis is smoked together with tobacco or other dried herbs. This procedure should be avoided to minimize the inhalation of smoke from burnt plant material. Many Dutch nationals smoke joints made from pure cannabis.

Pipes are superior to cigarettes in some situations in that they easily allow the person to smoke small amounts of pure high-grade cannabis. The percentage of tars in the smoke is reduced by condensation on the pipe walls. Pipes should be cleaned frequently. Water pipes are inferior to cigarettes and should be avoided.

Always use cannabis that is free of natural contaminants and adulterants.

Tests conducted by Dr Gieringer showed that vapourisers which heat marijuana to 180-190°C vaporised THC below the burning point of cellulose and other plant material also the production of polycyclic hydrocarbons was reduced. The best vaporizer delivered 10 parts of tar to one part of cannabinoids, cannabis cigarettes yielded a ratio of 13:1 (average), and water pipes an average of 27:1.

Thus, the best vaporizers achieved a performance ratio about 25% higher than the unfiltered cannabis cigarette, while water pipes were less favourable than cigarettes. The use of a filter in a cannabis cigarette was not advantageous since it not only filtered the tars, but also the cannabinoids. Indeed, the performance ratio was decreased by about 30% compared to the unfiltered cigarette.

In a new study Gieringer was able to demonstrate that combustion products were substantially reduced by using another vaporizer. The used device produced THC at a temperature of 185°C while completely eliminating benzene, toluene and naphthalene. Significant amounts of benzene began to appear at temperatures of 200°C, while combustion occurred around 230°C or above.

Traces of THC were in evidence as low as 140°C. Carbon monoxide and tars were both qualitatively reduced by the vaporizer, but were not quantificated in this study. However, a significant reduction of polycyclic aromatic hydrocarbons was assumed since vaporized cannabis emitted a thin gray vapor and the plant material was left with a green to greenish-brown "toasted" appearance, whereas the combusted sample produced thick smoke and turned to ash.

Just dug this up.

I think I remember this original research from some years back, the early days of UKCIA in fact and it's in our research library somewhere, but I don't have time to look now.

The original study, which was American, compared water filtered smoke with pure grass joints - not tobacco ones and I seem to remember they came the conclusion that a pure grass spliff delivered more of a hit than a bong, smoke for smoke.

But I agree, it doesn't sound very likely, bubbling the smoke through water is going to take out a lot of the heavy fraction tars.

A drug derived from marijuana, tetrahydrocannabinol (THC), formulated in sesame oil and encapsulated in soft gelatin capsules (Marinol®; Roxane), is currently available as a prescription drug for the treatment of two diseases or conditions. It is indicated for the treatment of nausea and vomiting associated with cancer chemotherapy, and for the AIDS wasting syndrome. The marijuana plant in smokeable form is available to ten patients in the United States for disorders such as glaucoma, spasticity, and the wasting syndrome. Each of these drug delivery systems, oral capsules and smokeable plant, has advantages and disadvantages, and each may be appropriate in particular circumstances. However, a drug delivery system that combines the rapid and reliable onset and ability to easily titrate an ingested dose (such as occurs by smoking marijuana cigarettes) with the least health risk (such as occurs by oral ingestion of capsules) would also be desirable. The use of water-filtered marijuana smoke, as produced by a water pipe, is one little-explored alternative. This article reviews some of the scientific work that has been done regarding the effects of water filtration on the composition and effects of marijuana and tobacco smoke.

While most of the research on water filtration has focused on tobacco smoke, the work with marijuana smoke has revealed that, except for their respective psychoactive components (nicotine and cannabinoids), both smokes share many common constituents and physical properties. Many of the results obtained from studies of tobacco smoke are applicable to marijuana smoke.

In the late 1970's, a group based at the University of Athens Medical School (Greece) conducted a series of chemical and pharmacological studies on marijuana and tobacco smoke.1-4 These scientists tested smoke that had been filtered through a water pipe and also tested the water itself, which contained both soluble and insoluble compounds. Chemical analysis revealed many different compounds in the smoke and in the water, as expected from the combustion of plant materials. The water did trap some THC, as well as other psychoactive compounds, however, most of the THC present in the marijuana passed through the water pipe unchanged. Pharmacological tests (in mice) revealed that some of the water-trapped marijuana compounds were responsible for producing catatonia and for suppressing spontaneous motor activity. In contrast, the water-filtered smoke itself did not affect spontaneous motor activity and did not induce catatonia, though it was richer in THC. These results indicate that water filtration removes some behaviorally active compounds in preference to others; this may be important when comparing the therapeutic effects of whole marijuana smoke to water-filtered smoke.

Research has shown that water filtration reduces both the amount of particulate matter and the number and quantity of toxic substances in the smoke that passes through it. In a 1963 study by Hoffman et al.,5 the water pipe was found to retain 90% of the phenol and 50% of the particulate matter and benzo-a-pyrene of the original tobacco smoke. In another study,6 tobacco smoke components that were passed through a water pipe showed only a minor hyperplasic reaction and no sebaceous gland destruction when they were painted onto mouse skin. (The application of substances to mouse skin to assess carcinogenic potential is a classic toxicological test; the induction of abnormal cell proliferation [hyperplasia] is a red flag.) In contrast, tobacco smoke condensate that was not water-filtered induced strong hyperplasia and complete sebaceous gland destruction when applied to mouse skin in the same concentration. Salem and Sami,7 also using the mouse skin test, showed that there was a significant reduction of carcinogenic potential in water-filtered smoke compared to the water remaining in the pipe i.e., the water-trapped material was more carcinogenic than the smoke that passed through it. Indeed, when analyzed by thin layer chromatography, two carcinogenic agents were identified in the water itself, while only one was identified in the water filtered smoke. Therefore, water filtration removes at least two known carcinogens that would normally be found in the smoke.

Recently, Dr. Gary Huber at the University of Texas and colleagues from Harvard's School of Public Health conducted a cellular toxicity study of marijuana and tobacco smoke.8 This research group showed that passing marijuana or tobacco smoke through water, or even exposing the smoke to a wetted surface of about 48 square inches, effectively removed substances (acrolein and acetaldehyde) which are toxic to alveolar macrophages. Alveolar macrophages are one of the major defense cells of the lung and are an important component of the immune system. When the macrophages were exposed to smoke that was not water filtered, there was a marked impairment of their capacity to kill bacteria. When the smoke was water-filtered, however, there was no reduction in the bactericidal ability of the macrophages, suggesting that marijuana smoke that has been passed through sufficient water will have less impact on the immune system than marijuana smoke that has not been water-filtered. This intriguing finding would be of particular importance when treating patients with the AIDS wasting syndrome.

The laboratory results discussed above parallel what is known from studying human tobacco-smoking populations. Thus, there is substantial epidemiological evidence that among tobacco smokers, those who smoke through a water pipe have a much lower incidence of carcinoma than those who smoke cigarettes or smoke a "regular" pipe or cigars.6,7,9,10

In summary, it appears that water filtration can be effective in removing components from marijuana smoke that are known toxicants, while allowing the THC to pass through relatively intact. The effectiveness of toxicant removal is related to the smoke's water contact area. Specially designed water pipes, incorporating particulate filters and gas dispersion frits would likely be most effective in this regard; the gas dispersion frit serves to break up the smoke into very fine bubbles, thereby increasing its water contact area. While individuals vary greatly in their smoking technique, state of health, dosing regimen, and so on, it seems that many patients could benefit from the use of water pipes to deliver THC. This would allow patients to titrate their dose easily while reducing the health hazard associated with smoke.

@Danzig - Are you sure that ratio is correct for tar, ie 10:1 vape and 13:1 cigs? Maybe I'm reading it incorrectly.. but it seems a little strange as there is never any visible tar when I clean my vape.

Answered my own question, old research, lots of problems as to validity - used water, higher cbd to thc percentage etc etc, also vapes have come a looong way since smile.gif

I smoke a bong without water in it and have done for many years, much prefer the taste and high compared to water being in there, plus I always seem to get a more chesty unmoveable cough if I smoke with water so perhaps there is some truth in it, who knows...
Plus the bong gets so messy when you smoke it with water, redeye pipe cleaner is a god send though! wink1.gif

One or two additional suggestions.

1] Smoke tobbacco and die. Pure weed or hash is always better. There are no healthy herbal or other mixes for cannabis.

2] If smoking grass has any ill effects on your lungs and windpipe stop and learn how to drink it instead, you don't want to to end up not being able to breathe. Former tobbacco smokers are advised not to smoke as it will almost certainly build on existing damage (as in my case).

2] Make sure you clean your grass of all sticks and seeds, it is well worth the time and less stress on the lungs.

3] If smoking a pipe then smoke through a piece of wet cloth on the mouthpiece for extra protection from tar.

4] Drink tea, coffee or hot choc while smoking and take a sip with every inhalation, this soothes the throat and helps to protect it.

5] Don't smoke Rasta stylee, (massive puffs of smoke) or you will get holes in your lungs. No need for joints the size of the M4, smaller the amount the better however you smoke it.

hi all, i was reading a study about the effects of smoking. the tar was a factor but the main stem of this study was more focused on the pesticides used on the tobacco plant when growing. plutonium is prescent in the smoke which causes hot spots in the lung so smoke outside if possible and avoid the tobacco.

that sounds about right. did you know they add well over a thousand chemicals in tobbaco, including gunpowder and various cancer causing chemicals. you can by chemical free tobaco hough. i belive Dunhill makes adidtive free fags(a bit rough for me though). my favourite is Natural Amercian spirit. you can buy in it sainsburys and such likes. it tastes alot better too... pinch.gif

found theis on wilkipedia its a summary of all the scientific studies/tests done on vapourizers mainly the volcano.

All Studies have found the release of harmful constituents dramatically reduced[7] or completely eliminated.[8] Substantial reductions were also found for the M1-volatizer.[9] However, a 1996 study[10] including two simple vaporizers still found ten times more tar in the vapor than THC, although this was nevertheless up to a 30% improvement compared to the best alternative smoking method.

The most recent study, published in the Journal of Psychopharmacology in May 2008,[11] investigated the acceptability and usefulness of intrapulmonary THC administration using a Volcano Vaporizer and pure THC instead of cannabis. Rising doses of THC (2, 4, 6 and 8 mg) were administered with 90 minutes intervals to twelve healthy males. Very low between-subject variability was observed in THC plasma concentrations, characterising the Volcano Vaporizer as a suitable method for the administration of THC.

In 2007, a study by University of California, San Francisco published in the Official Journal of the American Academy of Neurology[12] examined the effectiveness of a vaporizer that heats cannabis to a temperature between 180°C (356°F) and 200°C (392°F) degrees and found:
“ Using CO as an indicator, there was virtually no exposure to harmful combustion products using the vaporizing device. Since it replicates smoking's efficiency at producing the desired THC effect using smaller amounts of the active ingredient as opposed to pill forms, this device has great potential for improving the therapeutic utility of THC. ”

In 2006, a study performed by researchers at Leiden University, tested a Volcano Vaporizer with preparations of pure THC and found that:[8]
“ Our results show that a safe and effective cannabinoid delivery system seems to be available to patients. The final pulmonal uptake of THC is comparable to the smoking of cannabis, while avoiding the respiratory disadvantages of smoking. ”

When using plant material (crude flower tops), besides THC, several other cannabinoids as well as a range of other plant components including terpenoids were detected in the plant material. However, using pure THC in the Volcano Vaporizer, no degradation products (delta-8-THC (D8-THC), cannabinol (CBN), or unknown compounds) were detected by HPLC analysis. Also, a substantially larger fraction of the THC was delivered to the vapor by using pure THC.

Analysis of the vapor from the Volcano Vaporizer found that using multiple passes it delivered 36% - 61% of the THC in the sample.[7] A more recent study using pure cannabinoid preparations achieved a maximum of 54%.[8] For comparison, studies of cannabis cigarettes smoked via a smoking machine under varying conditions of puff duration and air speed found very similar efficiencies of 34% to 61%. Consequently, users can achieve the desired effect with a similar amount of material as when smoking.

In a 2001 study testing a device called the M1 Volatizer,[9] the researchers found that "it is possible to vaporize medically active THC by heating marijuana to a temperature short of the point of combustion, thereby eliminating or substantially reducing harmful smoke toxins that are normally present in marijuana smoke." The M1 Volatizer, produced THC at a temperature of 185°C (365°F), while eliminating three measured combustion products, benzene, toluene and naphthalene. Carbon monoxide and smoke tars were also reduced, but not quantified.

These positive results are in contrast to MAPS/NORML's previous studies into vaporizers which found less encouraging results, leading one to the conclusion that the effectiveness of vaporization varies greatly from vaporizer to vaporizer. A 1996 MAPS study[10] tested two simple vaporizer models against water pipes and filtered and unfiltered cannabis cigarettes (joints). The smoke produced by each was analyzed for solid particulates (tars) and 3 major cannabinoids. The various smoking methods were then rated based on their cannabinoid-to-tar ratio. The two tested vaporizers performed up to 25% better than unfiltered cannabis cigarettes (second best) in terms of tar delivery. However, both vaporizers produced more than ten times more tars than cannabinoids, which may partly be attributable to the low potency (2.3%) of the NIDA-supplied cannabis used in the study. Surprisingly, the same study found that water pipes (bongs) and filtered cigarettes performed 30% worse than regular, unfiltered joints. The reason was that waterpipes and filters filter out psychoactive THC with the tars, thereby requiring users to smoke more to reach their desired effect. The study did not, however, rule out the possibility that waterpipes could have other benefits, such as filtering out harmful gases such as carbon monoxide.

These studies have not measured the presence of toxic gases, such as ammonia, hydrogen cyanide and carbon monoxide, though previous studies have indicated unquantified decreases in carbon monoxide with vaporization.

Although vaporizers produce cleaner vapors than smoking, they do not completely eliminate respiratory irritation. A puff of strong vaporized cannabis will occasionally cause coughing. This however, could be due to THC itself, which is known to have a strong expectorant effect.

Vaporizer effectiveness

The wide range of results from tests of different vaporizers suggest that the choice of vaporizer is a major factor in determining extraction and delivery efficiency as well as the amount of harmful byproducts produced, or not produced, as in the case of a superior system. In Cannabis and many other medicinal plants, the components responsible for the aromatic nature of the plant will often vaporize at a low-end temperature in the range of extraction temperature values for all the bioactive components. In Cannabis, the temperature range across which the actives will vaporize is at least 56°C (132°F) starting at around 127°C (260°F) where only aromatic compounds of minimal bioactivity will release and going all the way up to 200°C (392°F) with the higher end of this range representing where the cannabinoids of higher bioactivity appear to be released. It is believed that both the total amount of actives delivered as well as the breadth of spectrum delivered per inhalation is critical in determining the value of the delivered dose and, in turn, systems that deliver the highest amount of actives and broadest spectrum of actives per inhalation are believed to be the most effective for medicinal applications: i.e. venturi enhanced extraction / convection based systems.

Proposed factors affecting output include:[7][8]

* Temperature
* Specimen density
* Weight, content of water and essential oils
* Consistency of material in the filling chamber
* Variety and potency of cannabis used
* Different preparations such as crude flower tops, hashish, hash oil, etc.
* Storage time of the vapor
* Proportion of THC exhaled (breathing technique)

Not all those have been scientifically tested. Research using the vaporizer found the delivery efficiency highest at around 226°C (439°F), falling to about half efficiency at 150°C (302°F) to 180°C (356°F) degrees depending on material.[8] The purest preparations produced the highest efficiencies, about 54% for pure THC versus 29% for plant material (female flower tops) with 12% THCA content. Besides THC, several other cannabinoids as well as a range of other plant components including terpenoids were detected in the plant material. Using pure THC in the Volcano Vaporizer, no degradation products (delta-8-THC (D8-THC), cannabinol (CBN), or unknown compounds were detected by HPLC analysis.[8]

The longer vapor is stored, the more of the THC is lost as it condenses on the surface of the vaporizer or the balloon. This loss may be negligible over a few minutes but may exceed 50% after 90 minutes.[8]

Interestingly, the Leiden Univsersity study[8] found that as much as 30%–40% of inhaled THC was not absorbed by the lungs and simply exhaled. However, they did not find large individual differences in the amounts exhaled.

I like the combination of tobacco with green. For me, it makes the hit more intense and delivers more pain relief. The tobacco is American Spirit which is chemical free.

I've adapted a bong ever so slightly. I smoke my joints through a water filled bong. Initially, I started to do this because of the grit situation, just to make sure. For me the draw feels smooth and it certainly cools it down if you use ice water. I got a plastic (travel) bong which cleans really well and is dishwasher safe. wink.gif

I ingest more Canna than I smoke to make sure my body is at a constant saturation point. clapping.gif Love Potion and Crystal Queen today! 13.gif Even leaving one of the twigs standing in yer cup of tea and coffee, for a few mins, kinda lightens the load. The biologist in me enjoys the thought of simple osmosis (or is that convection? I forget as I'm a bit mashed. Maybe it's both? unsure.gif) doing the job. Gotta love nature!

When I managed to quit smoking for three years I still smoked weed in a pipe, and to be honest my lungs never felt so good. In fact I felt no ill-effect on my lungs when I mixed weed with herbal (nicotine free) tobacco, so it just goes to show how bad regular tobacco is, I blame all the shit that they put in it.

Only reason they think a bong is more harmfull because it makes you cough becuase people dont clear the bong properly. they leave the bong when the tube is still full this makes you cough. Watch hashbean420's vids on youtube and see how he does it.

i smoke a bong with a small bowl.. much better and easier than a big hit.

I also found that smoking with herbal mix instead of tobacco removed a lot of problems i get with tobacco.

Get a carbon filter for your bong. It filters to the smoke and leaves your bong spotless so less crap in your lungs

for over 10 years i smoked pipes and bongs mainly. Last Christmas i got meself a handheld portable vaporizer( Iolite - formerly I-inhale) and it has changed everything. I constantly get a much more intense full-on HIGH than i ever got with the pipes/bongs/joints. They are the future I reckons. Clean and healthy and super super high cool.gif

What about combusted butane byproducts? Do they get in the vapor?

TBH I dunno. It does not feel or taste anything like butane. I have read some of the safety blurb the manufacturers have and (FWIW) they say that there is no butane threat - they would say that I know. But to me it does not seem to have any butane related ill effects.

And I forgot to say in me last post my herb lasts twice as long. And this has been the case for over 9 months now. Me level of stone has gone through the sky but me stash diminishes at a slower rate than when i was smoking de herb..
